PURPOSE OF THE UNIT STANDARD 
This unit standard is intended to enable the first responder in an emergency situation to react to health emergencies at an advanced level, until the arrival of more qualified emergency personnel.

Qualifying learners will be capable of:
  • Explaining the principles of advanced primary emergency care in all health emergencies.
  • Demonstrating an advanced level of preparedness to deal with potential emergencies.
  • Assessing and managing a complex emergency scene/disaster.
  • Explaining the applied anatomy and physiology of the human body systems and describing the emergency care management of disorders and diseases relating to each physical system.
  • Applying primary emergency life support for all age groups according to international and/or national protocols.
  • Explaining and managing shock.

    ASSESSMENT CRITERIA
     

    ASSESSMENT CRITERION 1 
    Applied anatomy and physiology of the various human body systems are explained in accordance with scientifically accepted terminology. 

    ASSESSMENT CRITERION 2 
    Common disorders and diseases relevant to each system of the body are identified and described in terms of signs, symptoms and emergency care management. 
    ASSESSMENT CRITERION RANGE 
    Include but not limited to:
  • Nervous system: Head injuries, epileptic seizures, stroke, transient ischaemic attack, poisoning.
  • Cardio-vascular system; cyanosis, angina, heart attack/myocardial infarction, heart failure.
  • Respiratory system: Drowning, poisonous chemical inhalation, croup, asthma, pneumonia, emphysema, haemothorax, pneumo-thorax, sucking chest wound, flail chest, pleurisy.
  • Infectious diseases/febrile diseases especially those prevalent in the community: Hyper/hypothermia, malaria, bilharzia, cholera, typhoid, gastroenteritis, childhood infectious diseases.
  • Musculo-skeletal system: Fractures, amputations, bone diseases, arthritis, dislocations, sprains, strains.
  • Digestive system: Poisons, dehydration/re-hydration, abdominal pain, bowel evisceration.
  • Urinary system: Urinary tract infections.
  • Reproductive system: sexual transmitted infections, emergency childbirth.
  • Integumentary system: Allergic reactions, burns, wounds.
     

  • SPECIFIC OUTCOME 5 
    Apply primary emergency life support for adults, children and infants according to current international and/or national protocols. 
    OUTCOME NOTES 
    Appropriately sized manikins and other supporting equipment must be used for the purpose of this assessment. 

    ASSESSMENT CRITERIA
     

    ASSESSMENT CRITERION 1 
    A primary survey is conducted; signs and symptoms are identified and recorded in terms of required protocols. 

    ASSESSMENT CRITERION 2 
    Methods of managing airway obstruction/choking are demonstrated. 
    ASSESSMENT CRITERION RANGE 
    Including but not limited to:
  • Head tilt-chin lift; Finger sweep; Heimlich manoeuvre/chest thrust/abdominal thrust; cardio-pulmonary resuscitation (CPR); Recovery position/lateral position.
     

  • ASSESSMENT CRITERION 3 
    One/two rescuer CPR is explained and demonstrated with the use of barrier ventilation devices, utilizing appropriately sized manikins. 
    ASSESSMENT CRITERION RANGE 
    Range of devices include but not limited to:
  • Mouthpiece, pocket mask, bag-valve-mask.
     

  • ASSESSMENT CRITERION 4 
    Recovery positions are demonstrated with and without spinal board and spider harness in accordance with the safety and comfort of the patient. 

    ASSESSMENT CRITERION 5 
    The patient is transported in terms of availability according to the conditions under which the incident occurred. 

    ASSESSMENT CRITERION 6 
    The need for oxygen therapy is explained, the administration is demonstrated and the dangers of open oxygen elaborated. 

    ASSESSMENT CRITERION 7 
    Pulse sites and pulse monitoring are explained and demonstrated in terms of on-going monitoring of the condition of the patient. 
    ASSESSMENT CRITERION RANGE

  • Carotid; radial; brachial; pedal; femoral; fontanelle; popliteal; temporal.

    Explain and manage shock.

As such, evidence should be judged according to the general principles of assessment described in the notes to assessors below. Learners who have met the requirements of this Unit Standard may apply for recognition of prior learning to the relevant Education and Training Quality Assurance (ETQA) body. The applicant must be assessed against the specific outcomes and with the assessment criteria of this Unit Standard.
